Your Personalized Care Plan Template to Take Charge of Your Health

This plan is divided into three key columns:

  • Goal: In this column, identify the specific health concern you're addressing. If helpful, include your motivation to feel better. Be clear and concise, for example you may want to reduce your lower back pain so you can play a sport with a friend, stop losing your hair so you can have the confidence to ask out your crush, or manage a specific chronic medical diagnosis so you can meet your grandchildren someday. Identifying what you want to improve and why creates a clear need for action.

  • Action: This column is your action plan! Outline the specific steps you'll take to address the issue. This might involve dietary changes, starting a new exercise routine, or following a doctor's recommendations. Browse our articles about your specific goals for self-care options and to help you find providers who can help you on your journey.

  • Result: As you implement your action plan, track the outcomes in this section. This is where you'll document how you're feeling, whether the chosen approach is making a positive impact, or if adjustments are needed.
